I hereby understand that my providing information to Gould Cooksey Fennell regarding my personal injury claim is a request for the firm to investigate and to represent my interests in said claim. I understand and agree that my submission of information in no way binds the firm and does not mean that the firm has agreed to represent my claim for personal injuries in any capacity, nor does it mean that the firm has agreed to act on my behalf in any capacity. I understand that a written contract for representation must be executed between myself and the firm before the firm will undertake any action or representation on my behalf. I further understand that any request by me to act on time sensitive matters has no legal effect and is not binding upon the firm until and unless the firm has agreed in writing to represent me in my claim.
